Release of Liability and Assumption of Risk 

PARTICIPANTS UNDER 18

- I, the parent or guardian, do hereby grant (athlete) permission to train with Knightmare Wrestling Academy at Rio Linda High School.

-I acknowledge, understand and agree that we are assuming risk of such injury, illness, disability or death by his or her participation in wrestling training and fitness activities. I assume full responsibility for his or her participation and give my permission to participate.

-In order that (athlete) may receive necessary medical treatment in the event of injury or illness, and a parent or guardian cannot be reached, I hereby authorize the Knight Wrestling Academy staff, coaches, clinicians or team parent to facilitate medical treatment for such illness or injury sustained during time in the training rooms, gym, or competition venue. Furthermore, Knightmare Wrestling Academy founders, principals, board members, owners and coaches, facility owners and Twin Rivers USD staff will not be held responsible for any injury or illness incurred while the minor is training at any of our above listed facilities, gyms or traveling to or from an event. 

- I verify that the minor does have primary medical insurance coverage. 

PHOTOGRAPH AND TALENT RELEASE AGREEMENT

 I hereby assign and grant KWC the right and permission to use and publish the photographs, film, videotapes, electronic representations, and/or sound recordings made of me or my child at all wrestling events/activities. I hereby release KWC, activity coordinators, volunteers, related parties, and other organizations associated with the activity from any and all liability from such use and publication. I hereby authorize the reproduction, sale, copyright, exhibit, broadcast, electronic storage, and/or distribution of said photographs, film, videotapes, electronic representations, and/or sound recordings without limitation of the discretion of the KWC. I specifically waive any right to any compensation I may have for any of the foregoing.
